We treat every young person and their family with respect, acknowledge their right to make choices about support that they need and enable young people to be as in control as they can be of the changes that they need to make.</p><p>We have a great opportunity for a motivated and proactive registered Social Worker who is committed to ensuring that the service supports young people and their families to make choices about their future in an individual and timely manner.  </p><p>To fully support young people as they transition into ASC we work closely with Children’s Disability Social Care, Education and Health Services. Knowledge, experience, and an understanding in this area of social work would be advantageous as will experience of working with young people with disabilities and their families. If you don’t already have this breadth of experience, we can train you. The Transition service is part of a large Council which actively promotes social workers CPD by offering regular training, appraisals, and supervision. </p><p>Our hybrid working arrangements allow for a mix of being home and office based throughout the week.
