We help people avoid going into hospital, be as independent as possible after a stay in hospital and prevent people from having to move into a residential home until they really need to.</p>\n<p>This post is part of a multi-disciplinary team, working together with therapists, nurses, social care and medical practitioners to promote independent living, support and plan their return back into the community. We can only do this because of our highly valued, skilled, and supported workforce.</p>\n<p><strong>Making A Difference as a Supported Living Worker in Community Participation Services</strong></p>\n<p>We are excited to offer several 1, 15 and 25 hour Support Worker vacancies in our Supported Living Service.</p>\n<p>Supported Living is a provider service that enables and supports adults with learning disabilities and complex needs in their own homes.</p>\n<p>Our overall aim and ethos are to promote positive fulfilling lives for Service Users, via social inclusion, choice, developing and promoting their independence in a person-centred way. They require support with direct personal care using safer people handling equipment where assessed. The role can often include supporting Service Users to manage their challenging behaviour by following risk assessments and risk management strategies, whilst maintaining and promoting their independence at all times.</p>\n<p>The client group we support require consistent intensive support, due to their level of learning disability and/or physical disabilities/behaviour. Some of our Service Users may experience problems in managing their complex behaviour; therefore, we adopt a multi-agency approach to help promote positive behaviour management, in the safest, ethical, and most appropriate way possible.</p>\n<p>This role will involve working from person centred support plans, risk assessments and the promotion of person-centred planning. Supported Living Workers will also be expected to follow all the policies and procedures that are in place e.g. safeguarding of vulnerable adult’s procedures and attend regular training.</p>\n<p>The role of Supported Living Worker requires total flexibility to support 25 Service Users living in 15 tenancies across the borough of St Helens. Shifts can include sleep in duties, waking nights and a variety of shift patterns e.g. an early shift can start from 7.30am and a late shift working up until 11pm. Some shifts mean a late shift followed by a sleep-in and an early shift the next day, which can equate to 24hrs over those two days. Shift patterns can change to meet the needs of the Service Users. Support Workers need to be flexible as you may need to work in various properties across the service. Shift work includes a proportion of weekend working and Bank holidays which are reflected on rotas.</p>\n<p>The hourly rate for this post is £13.26 to £13.47 per hour</p>\n<p>Applicants must have some experience of working with adults or children with a learning disability, and ideally possess an NVQ level 2 in Health and Social Care or equivalent and be willing to undertake all essential training to meet the Care Quality Commission Standards for the service. Sedgemoor Hub is a vibrant working environment. We have thirty beds and provide respite for people living with dementia, residential beds for people living with dementia and reablement beds. The role of Reablement Assistant is busy and satisfying. </span></p><p><span>You will work as part of multidisciplinary team with colleagues from health. Liverpool has a strong track record in promoting independence and enabling people to live well at home. Would you like to be part of a successful team working with other health and social care professionals to ensure we continue to provide a holistic assessment which has maximum benefit to service users and their families?</span></p><p><span><strong>The Role: </strong></span></p><ul>\n<li>As the Reablement Assistant you should have both knowledge and experience of holistic reablement and supporting people to live well with dementia </li>\n<li>As the Reablement Assistant you should be committed to providing the best support to achieve individual goals and maintain skills</li>\n<li>In your role as the Reablement Assistant you should be able to contribute to individual care plans, risk assessments and other essential records. Good communication skills are a must for this role</li>\n<li>Our Reablement Assistants must demonstrate an awareness of relevant CQC legislation, safeguarding and Health and Safety. All of which are of paramount importance to our service. Following guidance is crucial but you should also demonstrate an understanding of and commitment to person centred care</li>\n</ul><p><span>We want to ensure people here for reablement reach their reablement potential and return home wherever possible. People here for respite, feel like they have been on holiday and people living here feel it is their home and are supported to live well.</span></p><p><span>This position is 35 hours per week and shifts are: 8am -3:30pm and 2:30pm – 10pm across a three week rota. You will work two out of three weekends.
