The Dementia and Older People Team is a specialist multi-disciplinary team focusing on the needs of people living with the effects of cognitive impairment and dementia and older people with a functional mental illness. The Team provides specialist assessments and interventions embracing the Care Act, Mental Health Act, Mental Capacity Act, Safeguarding investigations and protection of human rights and civil liberties.</p>\n\n<p>We work to a model embracing self-directed care, utilising strengths-based approaches, co-production and promoting independence.
