As a Trust of schools, we are </span><em class=\"customHTML\"><span class=\"customHTML\">‘Always Learning’</span></em><span class=\"customHTML\">.</span></p>\n<p class=\"customHTML\"><strong class=\"customHTML\"><span class=\"customHTML\">Main responsibilities:</span></strong></p>\n<ol class=\"customHTML\">\n<li class=\"customHTML\">To establish and maintain effective working relationships with all professionals, pupils and parents associated with the school and to promote the service using a range of methods.</li>\n<li class=\"customHTML\">To support inclusive practice with vulnerable children and families., focusing on preventative and early intervention activities.</li>\n<li class=\"customHTML\">To act as Lead Professional when appropriate through supporting the Common Assessment process and to actively promote the use of the CAF with all agencies including the SENCO, Headteacher and Behaviour Support Team to improve outcomes for children and families and minimise the likelihood of referrals to Children’s Social Care which could result in children becoming subject to Child in Need and Child Protection Plans.</li>\n<li class=\"customHTML\">To keep confidential, accurate and relevant records and any other documentation pertaining to meetings/contact; assist in the process of quality assuring the CAF; track and monitor outcomes for children and young people and their family; review action plans and take corrective action if required.</li>\n<li class=\"customHTML\">To become a key member of the supportive team around the child/family/school in order to access additional support from other agencies and improve the welfare and life chances for identified families.</li>\n</ol>\n<p class=\"customHTML\"><strong class=\"customHTML\">Responsibilities</strong></p>\n<ul class=\"customHTML\">\n<li class=\"customHTML\">The post holder is responsible to the SENCO</li>\n<li class=\"customHTML\">The post holder will adhere to the school’s values and vision;
