- Case co-ordinating people
- Ensure our clients reablement journey is appropriate to meet their needs
- line manage a team of support workers
- Work in a fast-paced environment

- Gain strong professional relationships within the health and social care sector
- Full UK driving licence and own vehicle is essential to meet the travel requirements for this role.
- Business use car insurance is required as your personal vehicle will be used in connection with your employment when a pool car is unavailable.
For you to bring out the best in this challenging but fulfilling role, having a flexible approach to roster patterns that includes weekends and bank holidays will be essential.
