About the role

This full-time post is based in the Adult Social Care, Conquest Hospital team in Hastings.
You will need to be a qualified, registered (with Social Work England) and experienced Social Work professional. To carry out the role you will need a detailed knowledge of the Care Act (2014), Mental Capacity Act (2005), Equality legislation and the Deprivation of Liberty safeguards. You must be able to implement this legislation in your daily practice. You will also be expected to carry out the Local Authorities safeguarding duties to protect and mitigate risks to the individuals we serve. Experience of court of protection work would be an advantage in this role, however training is offered to successful candidates.
We have excellent relationships with local partners and hospital trusts. Together we work collaboratively to serve people to safely discharge from hospital back to their homes in the community. We promote the home first model and discharge to assess approach and believe with the appropriate safe level of care clients can maintain their independence and stay at home, safely where possible. We work closely with our reablement teams and our inpatient rehabilitation providers to maximise client’s independence and build confidence and positive outcomes for the client we work with. Where necessary we work with our local Integrated Commissioning Boards (ICB’s) to ensure people we serve get access to continuing care funding.
We are unique as a service in the fact that we have case responsibility for people who are inpatients and for those who have been discharged in the community requiring a Care Act assessment of need.

Your starting salary will be pro rata if the above position is part-time or term-time only.
