Your skills and knowledge will play a key part in shaping their future.</p><p>You will undertake responsibilities in supporting children and young people with transition support and targeted interventions. You will be a highly effective learning mentor who is committed in supporting children and young people whilst they are awaiting a school placement. The role will require you to have effective communication skills and an ability to work successfully with a wide range of people. This is an exciting opportunity for someone who has the skills to support service improvement and development.</p><p>We are looking for candidates who:</p><ul>\n<li>Can manage a caseload of children who require a range of interventions including SEND and targeted interventions in the home, IPS centre or virtually and monitor and review the effectiveness of these interventions.</li>\n<li>Provide advice and support for school transitions, liaising with school staff including SENCOs, pastoral staff, and colleagues in the Education Division regarding the pupil passport.</li>\n<li>Be the first line of contact with parent/carers, developing positive relationships with them and to meet with them to obtain pupil and parent voice.
