We do this working closely with a range of colleagues and other partners by focusing on improving the health and wellbeing of our poorest residents fastest to reduce health inequalities. </span></p> <p><span><strong>Your New Role</strong></span></p> <p><span>The Wellbeing Service supports individuals living in Portsmouth to modify behaviours, improve their health and life expectancy as well as reduce health inequalities. The service provides support to Portsmouth residents in a range of venues including community centres, GP practices and hospital settings. In addition, the service provides telephone/virtual support. Some of the work undertaken will be targeting Portsmouth's most deprived communities who unfairly suffer the greatest health inequalities.</span></p> <p><span>Within this role you will focus on supporting the Weight Management provision for clients, primarily those attending the Weight Management programme 'Healthier Me' in a range of community settings.  You will co deliver the weight management groups across PO1 - PO6 and assist in meeting, greeting and weighing clients.  This will include preparing resources and ensuring safe transportation and storage of resources and equipment for the sessions.  You will follow up with clients who do not attend sessions or require additional support as well as update all client interactions on the system. </span></p> <p><span>You may be required to support lower banded staff with the delivery of the programme as well as new referral processing and triage of clients as required.  In addition you will support the promotion of the service across the City. Work patterns typically involve 2 evening shifts per week. </span></p> <p><span><strong>What you'll need to succeed </strong></span></p> <p><span>With some experience of working in a customer facing role with clients (1-1's or group), you will have the ability to facilitate support.  You will have a knowledge and understanding of Portsmouth's demographic and of the wider determinants of health and how they can impact, as well as knowledge on how to engage people with support to address these.
