This team rarely need to recruit as once people join the team they don't want to leave. </span></p><p><strong>About The Job</strong></p><p><span><strong>Salary: </strong>£21,661 to £23,510 per annum</span><br><span><strong>Working pattern:</strong> 28 hours per week</span><br><span><strong>Contract type:</strong> Permanent</span><br><span><strong>Locations:</strong> Office base location flexible from one of our offices Horsham, Glebelands or Chichester. Hybrid working opportunities will be discusses at interview, with an expectation of 2 days per week in the office.</span></p><p><span>We are looking for a self-motivated, collaborative new assistant care manager to join our team. This is an excellent opportunity to make a significant difference to our service users lives, although challenging the rewards by far outweigh those challenges as you will always strive for a positive outcome. </span></p><p><span>No two days will look the same; you'll be working across Adur, Worthing and Midsussex with numerous colleagues and patients from all walks of life. You will need to take an organised approach whilst exercising your expertise with knowledge of policies and legislations to support those in need.</span></p><p class=\"Default\"><span>In this exciting opportunity, you will, under guidance and regular supervision, undertake strengths-based assessments (as directed by the Care Act 2014). You'll be utilising skills and knowledge to develop and implement a plan of support aimed at improving wellbeing and delivering outcomes for people who are D/deaf, deafblind, hearing impaired, blind (severely sight impaired) or partially sighted (sight impaired). The plan may include advice, information, the provision of assistive equipment or independent living skills. You will also be expected to carry out risk assessments and undertake assessments under the Mental Capacity Act 2007. </span></p><p><span>There will be some requirement to work flexibly out of office hours to assist customers, carers and families, and you will be required to travel independently throughout the County including to locations that are not always easily accessible by public transport. Excellent communication and interpersonal skills, including the ability to use a variety of information technologies.</span><br><span>5.
