The Dementia Post Diagnostic Support service is a countywide community service. The aim of the Dementia Post Diagnostic Support Service is to be the main point of contact for the person diagnosed and their carer during the entire dementia journey. The service provides appropriate emotional support, practical help, guidance and information to meet the needs of those entering the service, as well as facilitating access to well-being interventions which promote independence and quality of life.
Being a Community Dementia Support worker in the Carers Break and Engagement team is a rewarding and valued job which offers a great career pathway whilst making a real difference to the lives of those with dementia and their families and carers.
You will work in partnership with a wide range of organisations including GP’s and Primary Care clinicians and staff. The service enables identifying issues which lead to deterioration and we work with agencies to ensure a timely and pro-active response. Advanced Care Planning is a key element of the Dementia Post Diagnostic Support Service. A full and comprehensive induction will be given to all new staff.
We provide:
- Proactive support for the person with dementia and their carer/supporter
- Information, advice, and signposting.
- Emotional support and practical guidance
- Care co-ordination/planning
You will be required to have access to a car, and business insurance. Mileage is paid at 45p per business mile.
Previous experience isn’t necessary as full training will be given, but you will need to be patient, kind, and an excellent communicator with a desire to make a difference.

Your starting salary will be pro rata if the above position is part-time or term-time only.
