The service provides appropriate emotional support, practical help, guidance and information to meet the needs of those entering the service, as well as facilitating access to well-being interventions which promote independence and quality of life.</p>\n\n<p>Being a Community Dementia Support worker in the Carers Break and Engagement team is a rewarding and valued job which offers a great career pathway whilst making a real difference to the lives of those with dementia and their families and carers.</p>\n\n<p>You will work in partnership with a wide range of organisations including GP’s and Primary Care clinicians and staff. The service enables identifying issues which lead to deterioration and we work with agencies to ensure a timely and pro-active response. Advanced Care Planning is a key element of the Dementia Post Diagnostic Support Service.
