The role of the Reablement team is pivotal in supporting individuals to regain as much of their independence in daily living skills as possible, so they can do the occupations that are important to them, reflecting a person-centred approach. As a Social Worker in the Reablement team, you will work closely with the rest of the MDT Reablement team; our Reablement Officers, Occupational Therapists and Physiotherapist. You will also work closely with our internal teams in Operations, and also with our wider community teams, including our ILSS Providers. Your responsibilities will be to undertake Mental Capacity assessments, Care Act assessments and Support plans, for those residents with ongoing support needs. You will also identify Reablement potential and create Reablement plans for those residents who have Reablement potential. </p> <p><strong>Key Responsibilities:</strong></p> <ul>\n<li>Conduct timely Mental Capacity and Care Act assessments, and create support plans </li> <li>Collaborate and communicate with other professionals, both internally and externally to ensure a holistic approach</li> <li>Maintain accurate records and documentation in line with organisational policies and procedures</li> <li>Provide guidance to residents and their families /carer, that demonstrates independence and a strengths-based approach is promoted</li> <li>Identify potential for Reablement, and create Reablement plans tailored to meet individual needs, when identified</li> <li>Identify and act in a timely manner to any Safeguarding concerns</li> </ul> <p><strong>About You</strong></p> <p>We are looking for individuals who are passionate about promoting independence and improving the quality of life for our residents. The ideal candidate will have strong communication and interpersonal skills, the ability to work independently and as part of a team. You will need robust knowledge of Reablement practices, and be confident in your assessment skills and the application of the Care Act legislation. You should have a demonstrable strengths-based approach to your practice and a keen commitment to promoting positive risk management.</p> <p><strong>Qualifications and Experience: </strong></p> <ul>\n<li>A degree in Social Work and registration with Social Work England</li> <li>A good understanding of the key principles and Reablement </li> <li>Experience in completing Care Act assessments and creating personalised support plans</li> <li>Confidence in conducting mental capacity assessments</li> <li>Ability to work well under pressure, and to tight timescales </li> <li>Strong understanding of strengths-based practice</li> <li>Excellent communication skills and the ability to work effectively in a multi-disciplinary setting.</li> </ul> <p><strong>What we Offer You</strong></p> <p> We offer a supportive team environment, with direction to enable you to practice with confidence.
