You will play a hands-on role in creating tailored support plans that align with each person’s unique needs, talents, and aspirations. You’ll be supporting adults to rebuild confidence, develop essential life skills, and reconnect with their community through leisure activities, education, and independent living. No two days are ever the same, as you’ll be working in a variety of settings—whether it’s helping someone gain work experience, encouraging an individual to try a new hobby, or providing personal care as outlined in their Care and Support Plan.</p>\n<p> </p>\n<p>Your role will also involve building strong, trusting relationships with families, carers, and professionals from other agencies to create a truly collaborative approach. You’ll be the person others look to for insight and guidance at review meetings, where you’ll provide detailed, objective feedback to help shape the future care of the people you support.
