You will be supporting both Milton Grange, a 37 bedded short stay intermediate care unit as well as the reablement team within Joint Community Rehabilitation (JCR)which is countywide.This is a really rewarding role allowing you to work autonomously and be involved in the strategic development of the service being provided to our local population. You will see firsthand the positive impact your role is having in supporting people back to independence and being able to return home. This is a really exciting time for Directly Provided Services with a focus on growing and broadening access to our services.
