You will have strong social work values that lead your practice in helping young people to achieve the best outcomes possible. Alongside this you will play a significant role in working alongside a team of Social Care Practitioners to deliver outstanding assessment and support planning skills; that work across both Children’s and Adult’s services and legislation.  You will also apply strengths-based interactions within direct work to develop confidence and skills to achieve positive and sustainable outcomes for the future. You will work with both children and young adults and their families/carers to support them reach their potential. This will include undertaking a range of assessments including but not limited to; Child and Family Needs Assessment (s17), Care Act 2014 Assessments and Mental Capacity Assessments.</p>\n<p>PFA Social Workers will have good knowledge of legislation relating to adult social care and Children’s social care, and are able to implement this in their practice.  The ideal candidate should be willing to develop and be part of developing this new team alongside the other social workers and under guidance of the Team Manager.</p>\n<p>We are interested in applicants who ideally have experience of, and a passion to work with young people with various needs and are committed to providing a person centred and strengths-based approach to their work.</p>\n<p>Within the Preparation for Adulthood Team, we work with individuals between the ages of 14-25 within a variety of settings to maximise their independence and life opportunities. This role will require you to develop and imbed the PFA model within this new team. </p>\n<p> </p>\n<p><strong>Your role in our journey</strong></p>\n<p>As a Social Worker in our Preparation for Adulthood service you will have knowledge of Education Health Care Plans (EHCP), Specialist Education Needs Disability (SEND), safeguarding policy, Continuing Healthcare and any locally implemented transition pathways and processes.  You will develop creative and imaginative solutions to help young people achieve positive outcomes which enable young people to maintain their independence and live a happier, healthier life. With all the changes happening at Barnsley, there’s nowhere more exciting to hone your practice. Here, you’ll help individuals, families, carers and communities to make the right decisions for a safer, more independent future. Expected to undertake cases with support from Team Managers, Advanced Practitioners and other professionals as required and work to relevant procedures.
