The successful applicant will be employed in the Family Help Department of Wirral Borough Council and funded by NHS England. Our team seeks to improve outcomes and life experiences for those aged 0-25 with Learning Disabilities and/or Autism and we were established in September 2022.</p>\n<p>The NHS Long Term Plan outlines the commitment for all children and young people with a learning disability, autism, or both with the most complex needs to have a designated keyworker. The WALK team aims to provide a new way of multi-agency working. Our work is targeted specifically to support individuals (0-25 years old, inclusive) who are diagnosed with autism and/or learning disabilities to reduce their risk of requiring inpatient mental health support, prolonged stays in a mental health hospital, or placement breakdown. This may include working with individuals who face multiple needs and vulnerabilities such as being a ‘looked after’ or adopted, are in transition between services, or are at risk of youth offending. This role will include working holistically and closely with young people with autism and/or learning disabilities and their families, as well as professionals from across the system.</p>\n<p>The WALK team is guided by the Dynamic Support Database (DSD) function which is held and maintained by our team. The DSD facilitates an effective multi-disciplinary/multi-agency team around the child working to address risks and meet needs. There will be a caseload to manage in terms of supporting other professionals to collaborate and undertake agreed actions in a timely fashion, supported by your fellow keyworkers and the WALK Team Leader. Key workers will also help to organise and support Community (Education) Treatment Reviews when necessary for their cases.</p>\n<p>This post is a full-time position and will require the successful candidate to work with a range of ages up to 26 years old. Specific experience of mental health support when working with adults is a highly desirable attribute for this role. The successful candidate will be required to support individuals transitioning from children to adult’s services, ensuring their needs are still able to be met within their local community and facilitating good links during and after this transition.
