providing targeted advice and assistance to rough sleepers and individuals at risk of homelessness.</p>\n<p><strong>Key Responsibilities:</strong></p>\n<ul>\n<li>Offer housing advice and support to rough sleepers, in line with council policies and housing legislation.</li>\n<li>Conduct outreach in the community and work closely with partner organisations to ensure services are accessible.</li>\n<li>Carry out triage assessments and develop tailored support plans for clients, addressing physical and mental health, social care, and substance misuse issues.</li>\n<li>Collaborate with local initiatives, including the Rough Sleeping Initiative and Rapid Rehousing Pathway, to deliver comprehensive support.</li>\n<li>Maintain detailed case records and provide regular updates to ensure clients receive appropriate housing solutions.</li>\n<li>Undertake homelessness applications and related assessments to determine eligibility for assistance.</li>\n<li>Build long-term relationships with housing providers to prevent homelessness and secure alternative housing options.</li>\n<li>Actively contribute to team meetings, service improvements, and share specialist knowledge to support the team’s development.</li>\n</ul>\n<p><strong>Requirements:</strong></p>\n<p><strong>Essential Experience:</strong></p>\n<ul>\n<li>Proven experience in a housing-related support role, delivering advice and assistance.</li>\n<li>Experience working within a multi-agency environment.</li>\n</ul>\n<p><strong>Essential Knowledge:</strong></p>\n<ul>\n<li>Strong understanding of the causes of homelessness and the housing needs of individuals with complex issues.</li>\n<li>Knowledge of housing legislation, tenancy law, and the link between homelessness and health outcomes.</li>\n</ul>\n<p><strong>Skills:</strong></p>\n<ul>\n<li>Excellent communication skills (both written and verbal), with the ability to compile clear and comprehensive reports.</li>\n<li>Proficiency in IT and Microsoft Office applications.</li>\n<li>Ability to manage pressure effectively and work well within a team
