This service will include:</p>\n<ul>\n<li>Providing relevant information</li>\n<li>Home visiting and support</li>\n<li>Providing support to carers and families in meetings</li>\n<li>Planning, organising and hosting events as required<strong> </strong>\n</li>\n</ul>\n<p><strong>Hours:                        </strong></p>\n<p>15 hours per week per post over 2-3 days (Monday-Friday)</p>\n<p><strong>Location:</strong>                  </p>\n<p>Promise Inclusion office in Wokingham and other locations as required.</p>\n<p><strong>Salary:                       </strong></p>\n<p>£23,660 pro rata per annum (based on 35 hour working week).  This is under review.</p>\n<p><strong>Funding:</strong>                   </p>\n<p>The post is currently funded for 2 years, but further funding will be sought</p>\n<p><strong>Main duties include:</strong></p>\n<ul>\n<li>To communicate information on benefits and services to carers and families of children and young people with a learning disability and/or ASC by telephone, email or in person.</li>\n<li>To visit members/carers in their own homes and to build close relationships with those that need extra support.
