Gateway Team West
This role involves traveling throughout Hertfordshire to visit residents' homes and the office, providing crucial information and support to prevent, reduce, and delay the need for statutory services. You will work within a 12-week pathway to connect people with resources that empower them to manage their lives independently.
You will conduct assessments and reviews (including carers), develop personalized solutions, and coordinate support to maximize independence. Specialist training will be provided to assess functional ability and utilize technology and equipment-based solutions. You will collaborate with various partners while adhering to professional standards and legislation.
