It is for 30hrs and is  for 1 year fixed term.</p>\n<b>Main Responsibilities</b><br><p>You will be:</p>\n\n<ul>\n\t<li>supporting the Family Practitioner Leads to carry out the Local Authority’s SEND duties in relation to co-ordinating Education Health and Care (EHC) assessments</li>\n\t<li>producing EHC Plans and carrying out annual reviews</li>\n\t<li>closely with our children and young people with special educational needs and disability and their families as well as a range of qualified professionals in education, health and social care.</li>\n\t<li>develop collaborative and respectful relationships, promoting choice and independence and improve outcomes</li>\n\t<li>ensure that education, health, and care plans comply with legal, regulatory, ethical and social requirements in addition to the policies, standards and procedures of the authority.</li>\n</ul>\n<b>The Ideal Candidate</b><br><p>It’s a fast-paced role so you’ll need to be able to manage your own caseload independently and as part of the team. </p>\n\n<p>You will also:</p>\n\n<ul>\n\t<li>Need excellent written and verbal communication skills.</li>\n\t<li>Have knowledge of the SEND legislation plus, it’s important that you can reflect on practice and improve as a result. </li>\n</ul>\n\n<p>You can find out more about the role and the requirements in the job description attached.
