This role is directly responsible for managing the 0-25 service and bring positive outcomes for disabled children and young people within a diverse community.</p> <p>You will be required to operationally manage a dedicated team of Social Workers, Team Managers, Short Breaks Practitioners, Preparing for Adulthood Practitioners and other social care staff in the service, and to work collaboratively with Health, Education SEN colleagues, Children and Adults Services, and other partner agencies supporting the development of services for disabled children and young people and their families.  </p> <p>CYPDS service run a Duty Service, undertake assessments and hold case responsibility for children in need, supported enhanced short breaks provision, children supported under protection plans, children in care through to preparing for adulthood work and care leavers. The post holder will also be coordinating, planning and maintaining close links with external and internal key partners to CYPDS like schools, short breaks providers, Commissioning, Placements teams, children and adults mental health services.  </p> <p>The post holder requires knowledge of disability practice, a commitment towards integrated working and ability to work creatively with partners to embed Children’s legislation, SEND statutory guidance/ legislation and Adults Social care legislation related to disability.</p> <p>Within this role you will be responsible for social work and safeguarding practice within the service, timely and high-quality assessments, review and care planning, the on-going improvement of outcomes for disabled children and young people.
