The post holder will work as part of Brent Adult Social Care, promoting and advising on social prescribing approaches.</p>\n<p>The post holder will lead on promoting social prescribing in Brent, its role in self-management and in addressing health inequalities and the wider determinants of health. The role will support residents to take control of their health and wellbeing, targeting residents/communities with greatest needs and at risk of health inequalities.</p>\n<p><strong>The Person</strong></p>\n<p>You will be a dynamic, enthusiastic person with a good level of knowledge of a personalised care approach and have experience of working in health, Public Health or Adult Social Care.
