As part of this role, the post holder will take on the administrative management of the regular multi-disciplinary team meetings and agreed actions from these meetings.  The community inclusion worker will work with all health and social care professionals, as well as statutory/non statutory agencies, to provide a seamless, integrated service to our service users.</p>\n<p>The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS).
