We are working to build our own accommodation for resident with LD as well as looking to develop meaningful alternatives to day services.</p> <p>As a qualified Social Worker with two years post qualifying experience, you will hold a case load where you will implement short – long term interventions, utilising a strengths based approach to ensure that independence and well-being are promoted in the community. Social Workers will routinely undertake social care assessments, reviews and support planning, mental capacity assessments, Continuing Health Care decision support tools and safeguarding enquiries when necessary.
